QUESTIONNAIRE FOR GDPs ON THE APRIL 2006 CONTRACT

Welcome to the latest Challenge Questionnaire on the current NHS GDS Contract (2006).

We at Challenge are hoping to collect a mass of new information on the opinions of Dentists in General Practice on the effects of the 2006 Contract with a view to underpinning our work seeking to influence politicians of all parties.

We wish to assure all participants that none of their responses will be personally attributed.

 The first part of the questionnaire is just background about you and your practising circumstances. This will help in analyzing the information you provide.

All GDPs are welcome to participate , including those who have not experienced working in the GDS prior to the 2006 contract and also those who don’t hold a current GDS contract. As you will see, some of the questions are still applicable.
